I noticed in David Knape's demo he makes use of a 'top-level' or application mediator.
Is this a recommended best practice?

I am on a phone and can't check the contents of that Mediator, so I can't say anything about the practices he's using inside the Mediator. However having a Mediator for the application itself is certainly normal.
